Write and sign the termination
It must contain the number being cancelled, your identifying data (PESEL, or NIP/REGON for a company), your signature, and a clear statement that you are resigning.
Send it by registered letter
Dział Obsługi Klienta, Polkomtel Sp. z o.o., ul. Konstruktorska 4, 02-673 Warszawa. It can also be handed in at any Plus point of sale.
On a fixed term, count back 30 days from the end date
Notice later than that does not stop the contract converting to an indefinite one.
Expect the confirmation within 14 days
Polkomtel must confirm on a durable medium, giving the date it received your statement and the date the contract ends.
Keep the postal receipt until that confirmation arrives

- At least 30 days before your renewal date. Thirty days, counted from the first day of the billing period following the one in which the termination was submitted. A fixed-term contract auto-extends into an indefinite one unless notice is given no later than 30 days before the term ends.

- What happens after you cancel
- Plus wants paper, and it wants the paper to say the right things. A termination has to carry the NUMBER you are cancelling, YOUR IDENTIFYING DATA — PESEL for a person, NIP or REGON for a company — YOUR SIGNATURE, and an unambiguous statement that you are resigning. A letter missing any of those can be treated as not having been made. Send it by registered post to Dział Obsługi Klienta, Polkomtel Sp. z o.o., ul. Konstruktorska 4, 02-673 Warszawa, or hand it in at a Plus point of sale and keep the receipt either way. On Plus Mix, bok@plus.pl is also accepted. THE DATE THAT COSTS MONEY is 30 days before your fixed term ends. Give notice later than that and the contract does not end when the term does — it converts to an indefinite one on the same conditions, and you then have to serve the full notice period on top. Nothing about that is a penalty; it is simply months you did not intend to pay for. Put the date in a calendar the day you sign anything. Then watch for the confirmation. Polkomtel has to confirm on a durable medium within 14 days, and the confirmation names both the date your statement was received and the date the contract ends. Until it arrives you have a postal receipt and nothing else, so keep that receipt. Ending the contract inside a promotional term still means repaying the discount, proportionally to the part of the term left to run.
